OAKMONT OF ROSEVILLE’s first recorded visit in July 2021 was a pre‑licensing inspection conducted by Licensing Program Analyst Praveen Singh. The agency noted a well‑equipped facility—bathrooms with grab bars and non‑skid mats, hot water at 105 °F, adequate food supplies, and properly posted informational posters. The facility, then housing 79 residents under a change‑of‑ownership arrangement, had received a fire clearance on 24 May 2021 and was deemed ready for final licensing approval. In May 2022, Analyst Sarena Keosavang returned for a case‑management visit following two theft incidents reported on 24 March 2022: a resident’s wedding ring and chain went missing, and another resident was the subject of fraudulent bank activity. The facility’s internal investigation led to a police report (No. 2022‑15099). During the inspection the agency recorded no additional deficiencies, indicating that the incident had been appropriately addressed.

A July 2022 unannounced inspection highlighted infection‑control lapses. Analysts Keosavang and Jacob Williams observed kitchen staff preparing food and a contracted salon worker giving a manicure without masking, a serious concern given the facility’s COVID‑positive status. The agency cited personnel‑requirement and infection‑control deficiencies (LIC 9099‑D) and required a plan of correction by 26 July 2022, with the possibility of a civil penalty if uncorrected. The following year, on 27 July 2023, a repeat annual inspection by Keosavang found no deficiencies: rooms were properly furnished and lit, bathrooms clean, food supplies adequate, and all safety equipment—fire detectors, carbon monoxide alarms, and a recently serviced fire extinguisher—operational.
