The inspection was conducted to assess compliance with care standards related to urinary catheter care and prevention of urinary tract infections.
Findings
The facility failed to maintain Resident #86's urinary catheter bag off the floor on multiple occasions, contrary to facility policy and care plan directives, posing a risk for contamination and urinary tract infections. Staff interviews confirmed awareness that catheter bags should not be on the floor.

Visit Reason
The inspection was conducted to assess compliance with care standards related to resident incontinent care and infection prevention, as well as food safety and sanitation practices in the facility kitchen.
Findings
The facility failed to ensure proper glove use during incontinent care for a resident with a history of urinary tract infections, risking infection spread. Additionally, food holding temperatures were not properly monitored before meal service, and wet pans were improperly stacked, increasing risk of bacterial growth.

Certified Nursing Assistant used the same soiled gloves to place a clean brief on a resident, risking spread of infection.
Food holding temperature of fried chicken on steam table was below required 135 degrees Fahrenheit before meal service.

Visit Reason
The inspection was conducted due to allegations of abuse and failure to timely report suspected abuse, neglect, or theft to the State Agency, as well as concerns related to resident assessments, food safety, waste disposal, and infection control practices.
Complaint Details
The complaint investigation revealed that two of three abuse allegations were not reported to the State Agency within the required two-hour timeframe. The allegations involved physical abuse incidents occurring on 10/13/18 and 10/29/18, both reported late. The Director of Nursing confirmed the policy was not followed and stated there was no potential for harm as actions were taken despite delayed reporting.
