Visit Reason
The inspection was conducted as an initial licensure survey following a recent change of ownership on 6/1/25.

Findings
The facility was found to have multiple deficiencies including incomplete criminal background checks, lack of current relocation agreements, incomplete admission and negotiated service agreements, medication and diet administration errors, inadequate nursing assessments, maintenance and housekeeping issues, failure to maintain accurate resident records, and failure to conduct required fire and fuel-fired heating inspections.

Deficiencies (15)
Six employees did not have Department Criminal History and Background Checks after change of ownership.
Facility lacked current relocation written agreements for resident evacuation.
Admission agreements for six of seven sampled residents were not created and signed on or before change of ownership.
Facility was not maintained in a clean, safe, and orderly manner with damaged fences, stained windows, and damaged walls.
Residents did not receive medications and diets as ordered by physicians.
Nursing assessments were not conducted for residents experiencing changes in health status.
Medication destruction log did not document method of disposal.
Negotiated Service Agreements were not reviewed, updated, or detailed after change of ownership.
