Visit Reason
The inspection was conducted as an initial licensure survey for Overland Court Senior Living following a recent change of ownership and to assess compliance with regulatory requirements.
Findings
The facility was found to have multiple deficiencies including incomplete criminal background checks for employees, lack of current disaster/relocation agreements, missing admission agreements for residents, unsafe storage of toxic chemicals, medication management issues, incomplete resident assessments, failure to conduct required fire drills and inspections, inadequate food preparation practices, and failure to provide resident records timely.

Three of ten employees did not have a Department Criminal History and Background Check completed.
The facility did not have a current disaster/relocation agreement for emergency evacuation.
Admission agreements were not created and signed for six of eight sampled residents at the time of change of ownership.
Toxic chemicals were stored in an unlocked area accessible to cognitively impaired residents.
Residents were observed taking medications unsupervised without staff awareness.
Not all ordered PRN medications were available to residents at all times.
Medication destruction logs lacked required components and documentation.
Negotiated Service Agreements were not reviewed or updated after change of ownership and lacked clear service descriptions.
Pre-admission assessments were not documented for residents admitted on or after change of ownership.
Complaints from residents and family members were not documented by the administrator.
Staff records lacked required documentation including training and CPR/first aid certifications.
Annual fire alarm system inspection was not conducted; last inspection dated December 11, 2023.
Annual fuel-fired heating inspection was not conducted; last inspection dated March 3, 2022.
No fire drills had been conducted since licensing on June 1, 2025, failing bi-monthly requirement.
Residents were provided food that was often cold due to improper food handling and equipment issues.
