Inspection Report Summary for Park Plaza
In the 12/16/2021 annual visit, Licensing Program Analyst Michelle Reed documented that PARK PLAZA’s infection‑control procedures met all current state requirements. The facility posted COVID‑19 signage, maintained hand‑sanitizing stations throughout the building, and kept a 30‑day supply of personal protective equipment. Resident activity was observed in the lobby with proper mask use and social distancing. The administrator, Benjamin Davis, was reminded to review updated visitation, dining, and staff‑testing guidelines. The 11/02/2022 inspection by Edward Tapia mirrored the earlier findings. Staffing levels were adequate, and the facility had not previously seen exit‑seeking behavior from R1. Following the incident, a wander guard was to be installed and a care‑plan meeting is planned to transition R1 to memory care. The investigation concluded that no procedural failures contributed to the elopement, and no deficiencies were identified.
The most recent 12/14/2024 annual inspection, conducted by Alfonso Iniguez, found PARK PLAZA to be clean, well‑furnished, and largely compliant with Title 22 regulations. Fire safety equipment was operable, medication administration records had no discrepancies, and infection‑control posters were displayed. However, the report noted unpaid facility annual fees and cited a Title 22 violation that will incur a $100 fine per day until resolved. Despite this, the facility’s overall compliance with safety, sanitation, and resident‑care standards remained strong, underscoring a consistent track record of adherence to regulatory requirements with the exception of the administrative fee lapse.
