The inspection was conducted based on complaint number NJ00184292 to investigate compliance with 42 CFR Part 483, Subpart B, for long term care facilities.

The facility was found not in substantial compliance due to a deficient practice related to medication transcription for one resident. The facility implemented corrective actions including re-education of nursing staff, policy revisions, and ongoing audits to prevent recurrence.

Complaint # NJ00184292 was investigated and found to have a substantiated deficiency related to medication transcription errors.

Initial inspection for licensure of renovated long term care facilities.

No deficiencies were noted during the inspection of multiple resident rooms, common areas, and service rooms. The inspected areas may not be occupied until formal notification by the Certificate of Need and Licensing Division has been received.

A Life Safety Code Survey was conducted by the New Jersey Department of Health on 2/14/2023 and 2/15/2023 to assess compliance with Medicare/Medicaid participation requirements related to fire safety and life safety codes.

The facility was found to be in noncompliance with several Life Safety Code requirements including failure to perform semi-annual inspections of kitchen fire suppression systems, lack of audible and visible fire alarm notification in an enclosed courtyard, and missing fire sprinkler coverage in certain electrical rooms. Corrective actions were planned and completed by 3/31/2023.

Inspection of newly constructed and renovated long term care facility areas including resident units and common areas as part of a Life Safety Code Survey.

The facility was found to be in compliance with Medicare/Medicaid participation requirements and New Jersey licensure standards, with no deficiencies noted. The surveyed areas may not be occupied until formal notification is received.

The inspection was conducted in response to complaint #NJ 157108 to assess compliance with regulatory requirements.
