Resident 5 was discharged unsafely to a home with no running water, rats, and no heat. Facility failed to include primary physician in discharge planning. Staff were informed of unsafe discharge. Resident 5 is now in another skilled facility and doing well.

Facility failed to protect resident from physical abuse by another resident resulting in injury and prolonged pain. Failed to follow infection control standards and failed to employ a physical therapist. Multiple deficiencies related to care planning, infection control, abuse prevention, and clinical records were noted.

Facility failed to accurately document wound assessments and dressing refusals, failed infection control standards, failed to notify Ombudsman of hospitalizations, failed to provide bed hold policy notices, failed to accurately assess residents, failed to provide timely antibiotic treatment, failed to provide adequate nail care, failed to prevent falls, failed to ensure nurse call system function, failed to employ physical therapist, failed to coordinate hospice care, and failed hand hygiene and PPE protocols.

Facility failed to protect resident from sexual abuse by another resident. Failed to follow care planning, grievance follow-up, discharge notification, bed hold policy, assessment accuracy, ADL care, quality of care, fall prevention, respiratory care, therapy services, hospice coordination, infection control, hand hygiene, and nurse call system requirements.

Facility failed to have adequate staff to meet resident care needs timely, resulting in long call light response times and unmet needs. Multiple residents reported delays and lack of assistance. Staff reported chronic understaffing and high workload.

Facility failed to report complete COVID-19 information to CDC's NHSN during a required seven-day period, potentially causing more than minimal harm to residents.
