While the facility had received a fire clearance, the licensing evaluator noted that the resident rooms were not properly equipped to meet health‑safety standards. The facility was required to resolve these items by May 6, 2024, with a detailed plan of correction submitted.

A follow‑up case‑management visit in December 2024 revealed a separate but equally serious lapse. While staff and resident needs appeared otherwise met, the evaluator discovered that one employee did not possess the required criminal‑background clearance. Under the California licensing statute, this omission posed an immediate risk to residents. The agency cited the facility under 87355(b), imposed a $500 civil penalty, and mandated a plan of correction due by December 10, 2024. The report also documented that no resident was found without basic needs at the time of the visit.

In June 2025, the facility was subject to its mandatory annual inspection. Unannounced, the evaluator confirmed that all documentation—including admission agreements, medical assessments, TB screening, and medication records—was complete and current. Employee records satisfied all regulatory requirements, including first‑aid certification, fingerprint clearance, and CPR training. The physical plant, including temperature control, lighting, and water supply, was deemed adequate. No deficiencies were recorded, indicating that the facility had successfully addressed earlier safety and compliance issues and was operating in accordance with licensing standards at that time.

Across the three visits, a clear progression emerges: initial pre‑licensing shortcomings were identified and corrected, a staffing oversight was uncovered and remedied, and the facility ultimately demonstrated full compliance in its annual review. The pattern underscores the importance of ongoing oversight, timely corrective action, and rigorous staff screening in maintaining safe, compliant elder‑care environments.
