Inspection Report Life Safety Deficiencies: 8 May 15, 2025
Visit Reason
The Office of the State Fire Marshal conducted a fire protection inspection at the Colonial Court residential care facility on May 15, 2025.
Findings
The inspection identified violations related to sprinkler system testing and maintenance, smoke detector sensitivity failures, use of unapproved power adapters and extension cords, and the presence of a dry sprinkler head in a refrigerator. Follow-up testing and corrective actions were scheduled or completed for these issues.

Description
Forward flow testing of the backflow preventers shall be required; scheduled for June 5, 2025.
On the last fire alarm sensitivity test, 9 devices failed; follow-up testing report to be provided once replacement/recalibration is completed.
In room 2-9, an unapproved multiplug adapter was in use; removed.
In room 4-2, an extension cord was in use; removed.
A 1994 quick response pendent and 1993 quick response sidewall sprinkler heads have not been UL tested; UL documents received and testing recommended.
A dry sprinkler head dated 2018 was found inside the walk-in refrigerator and freezer.
After the last quarterly fire sprinkler inspection, the contractor did not replace the module covers for the fire alarm supervisory switches; replaced.
Key pads are posted at each exit and no codes are posted within 6 feet.
