At those times the home had no residents in care and had demonstrated compliance with basic licensing prerequisites, including staff training, emergency preparedness, and an up‑to‑date fire clearance.

The first required unannounced inspection, conducted on 22 January 2025, revealed several serious compliance gaps. Three staff records were incomplete or missing health screenings, and one employee (S3) had not been fingerprint‑cleared or formally associated with the facility; the agency imposed a $500 civil penalty for this violation. The facility also failed to meet criminal record clearance requirements for staff and had an improper arrangement of residents: a non‑ambulatory resident was housed in an ambulatory room, creating an immediate safety risk. Physical environment deficiencies were documented, including missing grab bars in a shared shower, half bed rails in three residents’ rooms, and hazardous cleaning chemicals stored in an unlocked kitchen cabinet. Fire clearance documentation was inadequate for the non‑ambulatory resident’s housing needs.

Despite these issues, the facility received a clear set of corrective action plans and a plan‑of‑correction deadline of 23 January 2025. The administrator agreed to remove the unassociated staff member, complete required fingerprint clearances, move the non‑ambulatory resident to an appropriate room, lock all hazardous chemicals, and update the fire clearance and emergency disaster plan. The agency noted that continued non‑compliance could trigger further penalties and that the facility must maintain public records of the inspection for three years.
