Inspection Report Summary for Safe Haven – Brentwood II

The California Department of Social Services’ Community Care Licensing Division conducted a series of visits to SAFE HAVEN ROSE GARDEN LLC (Facility Number 079201513) between March and May 2025. The March 17 visit was a COMP II telephone interview in which administrator Richard Grimesey, licensee Kevin Parker, and staff confirmed their understanding of the Code Title 22 regulations and submitted required documentation. At that time the facility had a capacity of six, a census of zero, and no clients in care.

On April 15 the licensing program analyst, T. Syess‑Gibson, performed an unannounced pre‑licensing inspection. The assessor toured every resident area, noted that lighting and safety devices such as carbon monoxide detectors and first aid kits were in working order, and identified several significant deficiencies. Hot water temperatures exceeded the 125 °F limit, a required grab bar was missing from a common restroom, mattress pads were not available for all beds, furniture blocked a passageway in the garage, and the backyard lacked a covered area, exit signage, and a properly locked medicine cabinet. Additionally, the dementia care plan failed to address behaviors such as ingestion of toxic chemicals and wandering. The analyst required that all corrections be completed by April 29, 2025, and that proof of these corrections be submitted to the Centralized Application Bureau.

A subsequent announced pre‑licensing visit on May 8 documented that the facility had addressed the hot‑water issue and several other deficiencies noted in the April report. The assessor noted that the corrections made since the April visit were satisfactory, and that the facility’s emergency and food‑supply preparedness had been maintained. While the initial inspection highlighted recurring safety and accessibility concerns typical of small residential care settings, the May inspection suggested that SAFE HAVEN ROSE GARDEN LLC had taken prompt corrective action to move toward compliance with licensing standards. The pattern of findings—particularly around environmental safety and documentation—underscores the importance of ongoing oversight for facilities with limited staff and a small resident population.
