The initial pre‑licensing visit in August 2021 confirmed that the property meets fire and safety requirements, holds a hospice waiver, and contains adequate private and shared rooms, staff quarters, ramps, and a well‑organized kitchen with locked sharps and medication cabinets. Subsequent unannounced inspections in August 2022, August 2023, and August 2024 found no deficiencies in fire protection, food storage, medication management, or resident living spaces. All common areas—including living rooms, dining rooms, bathrooms, and bedrooms—were furnished and maintained in good repair, with grab bars, hot‑water temperature, and call‑system functionality all meeting regulatory standards.

Across the four visits, the Administrator’s certification status evolved from an expiring conditional extension (expired 6/30/21) to a renewed active certificate (expires 4/29/2025). While the 2023 visit noted an expired certificate that had already been renewed pending issuance, no operational impact was reported. The facility’s census fluctuated between four and six residents, with one resident in the 2024 visit temporarily hospitalized; nevertheless, all resident files remained current, and staff records were complete. Fire extinguishers and alarms were inspected or purchased within the reporting period, and the perimeter fence and gates remained intact.

CCLD requested updates to several documentation forms (LIC308, LIC400, LIC500, LIC610) during each annual visit, but no corrective actions or violations were cited. The pattern of findings indicates that Serenity Home Care consistently meets licensing requirements, maintains rigorous safety and care protocols, and promptly addresses administrative compliance needs. No complaints or regulatory concerns emerged throughout the reporting period.
