Inspection Report Summary for St. Camillus Residential Health Care Facility

During the reporting period from August 1, 2021 to July 31, 2025, the facility received a total of 26 citations. These citations were divided into 14 Standard Health citations and 12 Life Safety Code citations, matching the state average for each category. The citations were issued during two on‑site inspections; no inspections without citations are reported in the summary.

The inspection that generated the largest number of cited items was the Certification/Complaint Survey conducted on November 20, 2023. This survey identified numerous deficiencies in Standard Health areas, including activities that meet each resident’s interests and needs, adequate ADL care for dependent residents, proper assistive devices for eating, development and implementation of comprehensive care plans, sanitary food procurement and preparation, Medicaid/Medicare coverage and liability notices, and overall quality of care. Deficiencies also appeared in resident rights and the resident call system. In addition, the survey documented multiple Life Safety Code deficiencies such as cooking facilities, corridor doors, self‑closing door devices, essential electrical systems, elevators, fire alarm system testing and maintenance, gas equipment storage, hazardous area enclosures, door maintenance and testing, portable fire extinguishers, and sprinkler system installation and maintenance.

Complaint metrics for the same period show that the facility received 138 complaints, equating to 54.5 complaints per 100 occupied beds, slightly above the statewide average of 53.2. Fifty‑three percent of complaints represented facility‑reported incidents, compared with 54.0 percent statewide. The Department of Health conducted 20 complaint‑related on‑site inspections at the facility, resulting in 10 complaint‑related citations and a rate of 3.9 citations per 100 occupied beds. When broken down by category, all complaint‑related citations were reported as zero for Administration, Dietary Services, Other Services, Physical Environment, Quality of Care, and Resident Rights, while the total complaints‑related citations reported was 0. This indicates a discrepancy between the stated total of 10 complaint‑related citations and the category subtotals, which do not sum to the stated total.

Enforcement actions taken against the facility between September 1, 2015 and August 31, 2025 include five stipulation and order determinations.
