Inspection Report Summary for Sunrise of Hermosa Beach

Sunrise Assisted Living of Hermosa Beach (Facility Number 198602887) was the subject of five separate CCLD evaluations between April 2022 and April 2023. In a case‑management visit on April 19, 2022, Administrator Janelle Odishoo met with Licensing Program Analyst (LPA) Martessa Brown to review documentation surrounding the death of Resident R1 on March 12. The analyst collected admission and medical records, toured the plant, and found no health‑or‑safety deficiencies, but noted that additional time was required to complete the investigation.

The same facility underwent an unannounced 1‑year compliance visit on September 11, 2022. Executive Director Ami Mehta and Reminiscence Coordinator Edith Duru guided LPA Ana Soto through the building, where every requirement—bathroom accessibility, fire safety, medication storage, and infection‑control stations—was met. No citations were issued.

A subsequent unannounced case‑management visit on March 22, 2023 revealed serious regulatory violations. LPA Jeremiah Randle found that the facility was retaining Resident R1, who had an unstageable pressure injury, a prohibited health condition. The administrator, Eric K. Mensah, had failed to request the required exception and did not meet qualifications for knowledge of applicable regulations. Two Type B deficiencies were cited: retention of a prohibited health condition and lack of administrator compliance with Title 22 regulations. The facility was issued a plan of correction and advised of potential civil penalties if deficiencies were not remedied.

On April 12, 2023, another unannounced 1‑year visit conducted by LPA Wendy Gibbs confirmed that Sunrise met all physical‑plant, food‑supply, and safety standards. The building was in good repair, rooms and bathrooms were clean and properly equipped, and staff had adequate PPE and infection‑control protocols. Overall, the facility demonstrates consistent compliance with licensing requirements, with the exception of the 2023 health‑condition lapse that prompted citations and required corrective action.
