The most extensive survey, issued on December 21, 2024, identified six violations, the highest count among all inspections in the period.

The December 21, 2024 survey highlighted deficiencies across several regulatory areas. Admission standards (487.4(a)) and admission and retention standards (1001.7(k)(2) and 1001.7(k)(5)) were cited, indicating issues with the facility’s processes for admitting and retaining residents.
