Beginning in May 2021, the department repeatedly met with the Director of Nursing, Lisa Codina, to confirm the removal of a former employee named “S1” and to verify that the facility had complied with the Decision and Order of Individual Exclusion. The visits followed up on multiple incident reports filed in October and December 2021 and on a self‑reported LIC624 incident involving Resident #1. Across all visits, staff interviews and resident observations yielded no cited deficiencies, and the facility was provided copies of the report and its licensing rights.

The most recent inspection in October 2023 served as the required one‑year compliance review. Amy Rodgers toured the complex, accompanied by Executive Director Timothy Jeffers and Resident Service Coordinator Carol Braun, noting that the four‑story building meets Title 22 requirements for fire safety, egress, and resident furnishings. The report highlighted proper use of call boxes, functional lighting, and compliant bathroom fixtures, reaffirming the facility’s commitment to safe, dignified care.
