Inspection Report Summary for The Palms at Bonaventure Senior Living

Palms at Bonaventure, a 740‑type residential care facility in Ventura, California, was the subject of an unannounced complaint investigation on July 9, 2025. The allegation that the facility lacked sufficient staff to meet residents’ needs was raised in a complaint received on June 2, 2025. Licensing Program Analyst Teresa Camara conducted the visit and interviewed the executive director, Brandy McCauley, along with the resident care coordinator, ten residents, and three staff members. Residents consistently reported that their care needs were met and had no complaints, while staff described contingency plans—calling on‑call personnel, having other staff stay late, or having management step in—to cover unexpected absences. The evaluator found no evidence that staffing levels fell below regulatory requirements and therefore classified the allegation as **Unsubstantiated**. No citations were issued, and the report was signed by the business office director on behalf of the ED.

On April 29, 2025, a COMP II (initial licensing) visit was conducted by analysts at the Centralized App Unit. The administrator, Brandy McCauley, and another licensed professional, Alan Spragins, verified their identities and confirmed their understanding of the California Code Title 22 regulations, admission policies, staffing requirements, and emergency preparedness. No deficiencies were identified during this telephone interview; the visit served to establish the facility’s initial licensing status.

A pre‑licensing inspection on May 16, 2025, evaluated the physical plant, resident rooms, fire safety clearance, and the proposed dementia and hospice services. The facility’s 121‑bed capacity and 90 private bedrooms met fire clearance standards, and all inspected rooms were properly furnished, well lit, and free of odor or safety hazards. No deficiencies were noted, and the facility was found compliant with applicable licensing requirements.

Across all visits, Palms at Bonaventure demonstrated that it maintains adequate staffing through established contingency protocols and that its physical and operational standards meet regulatory expectations. No regulatory deficiencies or citations were identified, and the unannounced complaint investigation concluded with an unsubstantiated finding, indicating that the facility’s care, safety, and compliance practices are currently satisfactory.
