Inspection Report Summary for The Terraces at Via Verde
The California Department of Social Services Community Care Licensing Division conducted two unannounced investigations at TERRACES AT VIA VERDE‑A MEMORY CARE COMMUNITY (Facility # 198603383) in San Dims, California. The July 21, 2022 visit responded to complaints that a resident (R1) sustained multiple pressure injuries, received medication and hygiene care in an untimely or incomplete manner, and that the facility was retaining a resident requiring a higher level of care. Detailed interviews with staff, residents, and family, along with review of medical records, hospice and home‑health documentation, and staff logs, found no evidence of neglect or failure to provide appropriate wound care or medication administration. The resident’s Stage 3 wound was being treated by hospice professionals, and the resident remained ambulatory and active. Consequently, all allegations were determined to be unsubstantiated.
In the August 30, 2022 inspection, allegations focused on inadequate assistance with hygiene and laundry, insufficient room cleanliness, lack of toiletries, and potential shortcomings in food service. The evaluator observed the facility, reviewed shower and laundry logs, and conducted interviews with six residents and five staff members. Residents and staff largely confirmed that showers and clothing changes were performed on a regular basis, laundry was processed daily, housekeeping was performed weekly, and toiletries were readily available. The evaluator noted that sticky‑note labels are used in the laundry area to prevent mix‑ups of residents’ garments, and that a property‑and‑valuables sheet is maintained for each resident. No evidence was found to substantiate any of the claims.
Across both investigations, a pattern emerges in which the facility maintains detailed documentation, scheduled housekeeping, and routine communication with residents and families. While complaints were raised regarding resident care and facility operations, the licensing evaluators concluded that the available evidence did not support the alleged violations. The reports remain on public record for the mandated three‑year period.
