Inspection Report Summary for The Woodlake Senior Living

The California Department of Social Services – Community Care Licensing Division first evaluated Sherwood Forest Senior Living on October 27 2020. In a pre‑licensing visit the facility’s Administrator, Ani Mkrtchyan, was interviewed over the phone and all eleven inspection domains were reviewed. The licensing analyst reported no deficiencies, indicating that the applicant met all initial regulatory standards and that the facility could proceed with its application pending approval from the Centralized Application Bureau.

A required one‑year inspection on January 18 2022, led by Licensing Program Analyst Patrick Shanahan, confirmed the facility’s continued compliance with health and safety requirements. The home—five bedrooms, two bathrooms, fully functioning smoke alarms, carbon monoxide detectors, and fire extinguishers—passed an infection‑control annual visit with no deficiencies noted. The administrator met the analyst, and the site was cleared to remain in operation.

On April 26 2023 a unannounced case‑management visit revealed significant fire‑clearance and building‑permit deficiencies. Licensing Program Analyst Abeye Duguma and Manager Naira Margaryan inspected physical plant changes that had not been properly notified to the licensing department. The facility had altered the layout of its bedroom spaces and converted a storage area into an additional bedroom without the required building permit, creating potential safety risks. Additional citations required an electrical permit for alarm switches and the removal of a fire‑door magnet kill switch. The department issued a plan of correction with a May 10 2023 due date.

An informal conference on May 2 2024 addressed ongoing compliance issues. The review panel—led by Licensing Program Manager Troy Agard and Analyst Gina Saucedo—highlighted that Sherwood Forest remains at risk of losing fire clearance because of unpermitted renovations, misuse of a backyard storage shed as office space, and improperly installed smoke detectors and electrical wiring. The facility was notified that continued deficiencies could lead to a non‑compliance conference and possible license revocation. The pattern across visits shows initial compliance followed by repeated violations related to unpermitted building alterations, fire‑safety, and electrical systems, underscoring the need for stringent adherence to permitting and fire‑clearance regulations to safeguard resident safety.
