Visit Reason
An unannounced, desk review Verification Visit was conducted on 03/28/2024 as a follow up to the recertification survey completed on 01/31/2024 at TSWV Chao Vang Center, LLC located in Milwaukee, WI.
Findings
TSWV Chao Vang Center, LLC was found to be in compliance with the Wisconsin Administrative Code DHS 105.14 for Adult Day Centers and the Home and Community Based Service (HCBS) requirements. No citations or recitations were issued.

Visit Reason
An unannounced recertification survey was conducted at TSWV Chao Vang Center Adult Day Care Center to assess compliance with Wisconsin Administrative Code DHS 105.14 for Adult Day Care Centers.
Findings
The facility was found out of compliance due to failure to provide required continuing education for staff, inadequate documentation of employee training, failure to follow food safety standards including hair restraints and hand washing, improper water heater temperature settings, and failure to provide annual training on resident rights.

Failed to provide the required 10 hours of continuing education for staff beginning with the first full calendar year of employment for 3 of 5 personnel files reviewed.
Failed to maintain documentation of orientation for 1 of 5 personnel files reviewed.
Failed to prepare and serve food according to professional standards for food service safety and failed to follow policy for hand washing when handling food in 2 of 2 observations.
Failed to set the temperature of water heaters connected to sinks at at least 140 degrees Fahrenheit and failed to maintain water temperatures at plumbing fixtures between 110-115 degrees Fahrenheit in 7 of 7 sinks observed.
Failed to provide annual training on resident rights for all staff employed at the facility.
