Inspection Report Summary for Wood Glen Hall

Wood Glen Hall, Inc. (Facility Number 421700457) is a 740‑bed community care facility located at 3010 Foothill Road, Santa Barbara, CA. In late summer 2022, the Department of Social Services, Community Care Licensing Division (CCLD) received a complaint alleging that staff were not associated with the facility. An unannounced inspection on 07/07/2022 found that 15 employees from a subcontracted home‑care agency and an additional eight facility employees had not been previously affiliated with the facility and did not possess the required criminal‑record clearance. The CCLD cited the violation of CCR 87355(e)(1) and assessed a civil penalty of $11,200. The investigation concluded that the allegation of staff affiliation was substantiated, underscoring a clear lapse in hiring and background‑screening procedures.

In early June 2023, a subsequent complaint was investigated by Evaluator Brian Phillips. The allegation concerned medication assistance and meeting residents’ needs, alleging that a private caregiving company had taken over medication dispensing for a wing of the facility. The report reviewed the contractual arrangement, which stipulated that the company’s employees were licensed Home Care Aides and were vetted by the California Department of Social Services. No evidence supported the claim that medication duties were improperly delegated, and the allegation was deemed unsubstantiated.

A separate complaint investigated on the same date addressed the administrator’s communication with residents’ representatives during a flu‑like outbreak. Although three residents exhibited symptoms and were isolated, the administrator had informed the responsible parties of the precautionary measures. The report found insufficient evidence to substantiate the claim that communication was lacking, and the allegation was also marked unsubstantiated.

The pattern emerging from these inspections highlights recurring challenges with staffing compliance and external agency oversight, while administrative communication issues were not corroborated. The substantiated findings emphasize the need for stricter enforcement of background‑screening requirements and clearer documentation of staff affiliations to protect resident safety and maintain regulatory compliance.
