WESLEY PALMS, a 740‑type community care facility located at 2404 Loring Street, San Diego, has demonstrated overall compliance with California’s licensing standards across multiple visits between August 2021 and April 2023. In the August 5, 2021 Case Management “Other” visit, the team conducted a technical‑assistance walk‑through of the facility’s COVID‑19 mitigation plan—covering disinfection, testing, vaccination, screening, and PPE usage. The visit resulted in no deficiencies cited, indicating that the facility’s pandemic protocols were both documented and operational.

The most significant compliance issue emerged in the unannounced Case Management visit of September 14, 2021. The Licenses Program Analysts followed up on self‑reported incidents from December 2018 involving a staff member (S1) who stole residents’ credit/debit cards and used the information for personal expenses. The investigation confirmed that S1’s fraudulent activity constituted a health and safety violation under Health and Safety Code 1569.58, leading to a citation and a Plan of Correction. S1 was later sentenced to four years in county prison in May 2021. This incident remains a historical deficiency; the facility has not reported similar incidents since.

Subsequent visits in January and April 2023 focused on resident safety incidents reported by the facility itself. In January, a resident (R1) inadvertently left the facility and was later located unharmed; the visit confirmed that the resident’s Absentee Notification Plan was properly followed and that no deficiencies were identified. A parallel January visit addressed infection control, reviewing the facility’s COVID‑19 mitigation measures, and likewise found no violations. In April, the facility self‑reported missed antibiotic doses for R1; the visit noted that the medication error did not result in adverse health effects and that staff promptly addressed the issue with the resident’s responsible party and physician. No deficiencies were cited.
