Inspection Report Summary for Raya’s Paradise, Inc IV
RAYA’S PARADISE, INC. (Facility No. 197604445) in West Hollywood underwent three unannounced, one‑year required inspections during 2021‑2023, all conducted by California Department of Social Services Community Care Licensing (CCLD) analysts. The 2021 visit was led by Executive Director Ruby Cruz and the 2022 and 2023 visits were conducted by Licensing Program Analysts LaQueena Lacy and David España, respectively, who met with Administrator Moti Gamburd and, in 2023, also with Administrator Brain Rosales. Each inspection confirmed that the facility maintained its approved mitigation plan, temperature controls, hand‑washing and mask‑wearing protocols, and required signage, with no health or safety concerns identified.
Across the three visits, the agency observed consistent adherence to fire safety, infection control, and resident‑care standards. Fire extinguishers, sprinklers, and alarms were tested and found operational; the kitchen and laundry areas were clean and properly secured; medication and sharps storage was locked and inaccessible to residents; and all bedrooms, bathrooms, and common areas met the required lighting, furnishing, and accessibility guidelines. Water temperature measurements in the bathrooms were high but within the documented range and accompanied by grab bars and non‑skid mats. The 2023 inspection noted a non‑operational water fountain but no other deficiencies. No complaints or citations were recorded in any report.
The pattern of findings indicates a consistently compliant facility that successfully meets CCLD’s regulatory requirements. Each annual inspection concluded without deficiencies, reinforcing the effectiveness of RAYA’S PARADISE’s mitigation and safety protocols. The repeated, thorough reviews by different analysts demonstrate robust oversight and a stable compliance record for this West Hollywood group home.
