Inspection Report Summary for Meadowlark Gardens II
The July 2021 inspection, conducted by Licensing Program Analyst Shobhana Frank, was an unannounced, required annual review of Meadowlark Gardens IV, a single‑story home licensed for six residents, four of whom are non‑ambulatory. Frank noted that the facility’s physical plant, safety features, and operating conditions fully met CCLD regulations. The residence was clean, well‑maintained, and equipped with necessary safety devices such as grab bars, non‑slip mats, and operational smoke and carbon‑monoxide detectors. COVID‑19 protocols—including a visitation station, hand sanitizer, and visitor logs—were observed, and all resident records, staff files, and documentation of resident rights were up to date. No deficiencies were cited.
In the June 2022 visit, also led by Shobhana Frank, the same unannounced annual inspection confirmed continued compliance. The evaluator reviewed resident and staff files, medication storage, kitchen and food service, and emergency disaster plans. Facility resources such as a 2‑day perishable supply, a 7‑day non‑perishable stock, and locked cabinets for hazardous substances were deemed adequate. Physical safety items—including fire extinguishers, functional detectors, and a charged first aid kit—were inspected and found in proper condition. Again, the report recorded no deficiencies in any area evaluated.
The September 2024 inspection, conducted by Licensing Program Analyst Michael Tea, reiterated Meadowlark Gardens IV’s adherence to regulatory standards while highlighting a hospice waiver for four residents. The evaluator observed routine activities such as bingo, exercise sessions, and pet‑therapy visits, and confirmed that staff and administrator certifications were current. Medication was securely stored in a locked cabinet, and all safety equipment, including fire extinguishers and a first aid kit, was fully functional. As with previous visits, no deficiencies were identified.
Across all three inspections, a consistent pattern emerges: Meadowlark Gardens IV maintains a safe, compliant environment for its residents, demonstrates proper staff training and documentation, and effectively manages its physical plant and emergency protocols. Each report, signed and made available for public review for three years, reflects ongoing regulatory compliance and a commitment to quality care.
