We are nurse owned and operated and collaborate closely with hospice, home health, and primary care providers to ensure a comprehensive and seamless care plan. We have onsite primary care visits, lab work, urgent care, and all medications can be delivered to the facility. This coordination allows us to support both the medical and personal needs of our residents. We had a daughter who had been her mother's constant caregiver and advocate for years, so consumed by worry at the previous facility that she couldn't leave her mom's side. After her mom moved to Heaven's Hands, she stayed involved, but something slowly shifted. She told me she finally felt like her mother was truly known and cared for. Last year, she took her family to Costa Rica. First real trip in years. She came back and told me it was a success and that she'd actually been able to enjoy it, knowing her mom was in good hands. That meant everything to me. That's what we're here for: not just caring for our residents, but giving their families permission to breathe again. What makes us different is that we're a real home, small by design, owned and operated by an RN and an MSW, with consistent staff who know each resident deeply. Not a facility. A home. Although we have extremely high care standards, our last State inspection received a perfect score with no deficiencies found.
