SAGE MOUNTAIN SENIOR LIVING FACILITY (Facility Number 565802462) was the subject of four unannounced complaint investigations during 2021. Two of the five complaints were substantiated, highlighting ongoing maintenance and sanitation deficiencies, while the other two were found unsubstantiated, indicating the facility’s ability to address certain operational concerns.
During the first visit on May 27, 2021, investigators determined that Resident 1’s bedroom was malodorous due to pet urine. The state cited a violation of CCR 87303(a), which requires the facility to maintain a clean, safe, and sanitary environment. A Plan of Correction was issued for June 11, 2021, requiring the administrator to eliminate the odor and allowing a follow‑up visit. A month later, on June 21, 2021, a second investigation revealed that the same room had not been cleaned and that it was in disrepair—stains from the resident’s dog and physical damage to walls and fixtures. This too was substantiated under CCR 87303(a), with a corrective deadline of June 21, 2021 and an immediate plan to replace carpet and repair walls.
The subsequent inspections on July 22 and September 9 addressed complaints unrelated to sanitation. In July, staff were alleged to have neglected to empty laundry‑room trash on a regular basis. Interviews with residents and staff, as well as a plant tour, found no evidence of a lapse; the allegation was therefore unsubstantiated. In September, an allegation that facility staff were preventing the licensed therapist‑counselor‑of‑care (LTCO) from holding confidential meetings with residents was likewise unsubstantiated after staff interviews and documentation review revealed no impediments.
A clear pattern emerges: repeated failures to maintain a resident’s bedroom, despite earlier notices, suggest a systemic issue with cleaning protocols and quality assurance. While the facility has addressed the trash and LTCO concerns satisfactorily, the repeated, uncorrected problems in Resident 1’s room raise questions about the effectiveness of corrective actions and oversight. Continued monitoring will be necessary to ensure that maintenance standards are met and that similar deficiencies do not recur.
