Most complaint investigations resulted in unsubstantiated allegations or zero deficiencies, suggesting generally compliant operations. The two substantiated complaints reflect serious concerns that were remediated. The July 2021 elopement and safety complaint, substantiated, documented failure to ensure a safety alarm on an exit door could be heard throughout the facility, failure to provide supervision and implement interventions for a cognitively impaired resident with exit-seeking behavior, and failure to submit an incident report for a missing resident who had exited without staff knowledge.

A plan of correction was due September 10, 2021. The October 2020 abuse policy and infection control complaint, also substantiated, found failure to maintain CDC personal protective equipment guidelines and failure to thoroughly investigate and document a physical abuse allegation by a staff member, allowing the accused to continue working. Corrections were confirmed by December 13, 2022 revisit. The October 31, 2025, complaint investigation into medication misappropriation and staffing allegations found 2 deficiencies for food labeling and coverage failures in refrigerators and freezers, requiring a plan of correction.

The December 15, 2025, sexual assault protection complaint found zero deficiencies and was unsubstantiated. The June 2024 HIPAA complaint found zero deficiencies and was unsubstantiated. The October 2024 elevator malfunction complaint found zero deficiencies; residents could evacuate safely with assistance. All renewal/re-licensure surveys from August 2019 through May 2023 found zero deficiencies.

The March 2019 medication misappropriation complaint found zero deficiencies and was unsubstantiated. No fines or license suspensions appear in the available record.

Operator is Baptist Village of Oklahoma City. Yukon is west of Oklahoma City in Canadian County; Integris Canadian Valley Hospital is the primary nearby acute-care resource.
