Six deficiencies were cited: food items not covered or labeled with open and use dates, failure to provide sufficient staff for 14 residents not capable of self-preservation, failure to provide registered nurse supervision for eight residents for skilled nursing interventions, failure to ensure correct preparation for non-crushable medications, failure to ensure two staff received abuse training within 90 days of hire, and failure to conduct registry screening and background checks for two staff upon hire.
A November 3, 2025, complaint investigation addressing residents’ right to privacy, emergency evacuation staffing, freedom from abuse, and elopement prevention found no deficiencies. A November 16, 2025, complaint investigation substantiated failure to prevent resident elopement resulting in immediate jeopardy. One resident eloped using a secured employee code for the rear delivery door and was found on a busy street despite prior knowledge of the resident’s exit-seeking behavior. Immediate jeopardy was lifted after verification of corrective actions including enhanced supervision, door security upgrades, staff training, and medication adjustments with completion due December 17, 2025.
