A February 12, 2024, relicensure survey cited three deficiencies: failure to ensure leftover foods in the refrigerator were dated with preparation date, staff monitored temperatures and chemicals, and hair restraint policies for facial hair were in place; failure to ensure resident assessments were coordinated and signed by a registered nurse, physician, and resident or representative for five of eight sampled residents; and failure to ensure contracts were fully completed for two of eight sampled residents and failure to implement a policy to prevent cross contamination for shared blood pressure cuff use. An October 16, 2025, relicensure survey with complaints cited four deficiencies: open food items not labeled with open or use by dates and incomplete temperature and dishwasher sanitization documentation; comprehensive assessments not completed within 14 days of admission for one resident; assessments not coordinated and signed by a registered nurse or resident’s personal physician for one resident; and staff not educated on abuse within 90 days of hire for one employee. Substantial compliance was achieved upon a November 17, 2025, revisit.

A June 11, 2025, license renewal found no deficiencies. The facility’s occupancy has remained between 36 and 46 residents in recent inspections. No fines or license suspensions are recorded.

The facility provides assisted living and memory care services. Licensed capacity is 50 beds with current occupancy of 42 residents (84 percent). Nursing hours 28 percent above Oklahoma benchmark. Operator is TP Stillwater Operations, LLC.
