The screening logs showed inconsistent documentation and the policy did not specify screening methods or frequency. This deficiency represented potential for more than minimal harm with no actual harm identified. An April 12, 2023, relicensure survey cited three deficiencies: failure to ensure resident assessments were coordinated and signed by a registered nurse or resident’s personal physician for two of eight sampled residents, failure to ensure resident assessments were signed by the resident or resident representative for two of eight sampled residents, and failure to develop care plans to reflect resident assessments for two of eight sampled residents.

A March 1, 2025, license renewal found no deficiencies, certifying continued operation with a maximum capacity of 52 beds. Administrator Ashley Miller. The facility’s occupancy has remained between 24 and 29 residents in recent inspections. No fines or license suspensions are recorded.
