Lambs Creek West AFH is a small, owner-occupied Adult Family Home licensed to serve frail and elderly adults, adults with developmental disabilities, and a primary focus on physical disabilities. We provide individualized, home-like care in a calm and respectful environment and are particularly suited to Members who benefit from consistent routines and individualized physical assistance, including those who use wheelchairs or require assistance with mobility, transfers, personal cares, or adaptive equipment. Lambs Creek operates primarily with a single-staffing model and therefore admits and retains Members whose needs can be safely and appropriately supported within that model. Our goal is to create a home—not simply provide a placement—where Members are known, respected, supported, and able to continue living meaningful lives.
