The July 2022 case‑management visit uncovered a criminal‑record‑clearance lapse: two staff members, Princess Diaz and Miguel Can, had not obtained the required California clearance, resulting in a $100 civil penalty for Diaz and a $500 penalty for Can. Both citations were cleared on the same day after the licensee, Stephanie Klein, submitted the necessary clearances. The November 2022 inspection repeated the infection‑control focus and found no new deficiencies, with all rooms and common areas compliant with Title 22 regulations.
