Bayshire Carlsbad, a 741‑type residential facility, underwent a series of licensing evaluations that highlight a generally compliant physical and operational environment but a persistent challenge with medication administration. The initial change‑of‑ownership review in May 2021 confirmed that the new administrator and owner understood Title 22 regulations, and the June 2021 pre‑licensing visit found the facility’s infrastructure, sanitation, fire safety, and hazardous‑substance storage to be in order, with no compliance deficiencies noted. A subsequent case‑management visit in September 2021 focused on COVID‑19 mitigation and personal protective equipment protocols; the evaluator reported no deficiencies and offered technical assistance.

The pattern of concern began in October 2022 when two self‑reported medication errors prompted an unannounced case‑management inspection. The first error involved a resident receiving another resident’s medication bundle, which caused no adverse effects but violated physician orders.
