Earlier inspections showed a pattern of issues related to medication reassessment, residents’ rights, and updating individual service plans, with a substantiated complaint investigation in February 2024 that included a $700 forfeiture. Inspectors cited failure to ensure quarterly reassessment of psychotropic medications and limitations on residents’ self-determination due to locked doors, as well as incomplete updates to service plans reflecting residents’ changing needs.
