Resident R1 experienced repeated falls, suicidal ideation, and elopement behaviors that were not followed by timely reassessments or updated care plans. Documentation gaps included the absence of communication with R1’s responsible party regarding suicidal thoughts, lack of ongoing monitoring after each incident, and failure to sign updated assessments. The evaluator noted that the facility’s neglect of these duties resulted in a $500 civil penalty for bodily injury and violated several Health and Safety Code sections. This visit highlighted a pattern of inadequate staff qualification and deficient resident‑care protocols, contrasting sharply with the earlier compliant inspection. The repeated licensing interactions suggest the facility’s operational issues were escalating, requiring continued oversight to ensure resident safety and regulatory compliance.
