Visit Reason
On-site annual compliance inspection of an Assisted Living Home conducted on December 4, 2025.
Findings
Three deficiencies were found related to tuberculosis infection control activities and personnel fitness documentation. The inspection identified missing TB risk assessments for staff and a resident, and incomplete documentation of previous employer reference checks for personnel.

R9-10-113 — The chief administrative officer failed to ensure tuberculosis infection control activities were established, documented, and implemented for one of three personnel reviewed, missing the required TB Risk Assessment/Signs & Symptoms Screening. This posed a potential TB exposure risk.
R9-10-803 — The governing authority failed to ensure compliance with A.R.S. § 36-411.C.1 by not documenting good faith efforts to contact previous employers for three personnel reviewed. This posed a risk as required information to determine fitness to work was not verified.
R9-10-807 — The chief administrative officer failed to ensure tuberculosis infection control activities were established, documented, and implemented for one of two residents reviewed, missing the required TB Risk Assessment/Signs & Symptoms Screening. This posed a potential TB exposure risk to residents.
