On-site complaint investigation of complaint #106508 and annual compliance inspection at an Assisted Living Home, conducted 3 April 2025.

Complaint Details
The following deficiencies were found during the on-site compliance inspection and investigation of complaint #106508 conducted on April 3, 2025.
Findings
The inspection found four deficiencies related to tuberculosis training, verification of employee status on the adult protective services registry, use of restraints on a resident, and inadequate exit alarm systems. Plans of correction were provided for all deficiencies.

R9-10-113 — The facility failed to provide annual training and education on recognizing the signs and symptoms of tuberculosis to all employees, as documentation only showed TB screening without evidence of training.
R9-10-803 — The governing authority did not verify that employees were not on the adult protective services registry as required by law, lacking documentation for three employees.
R9-10-810 — The manager failed to ensure a resident was not subjected to restraint, as a mattress and dresser were used to restrict the resident's movement despite documentation indicating restraints were not needed.
R9-10-815 — The manager failed to ensure that the patio door alarm controlling resident egress was functional, posing a risk of unmonitored resident exit.

On-site annual compliance inspection of an Assisted Living Home conducted on July 21, 2023.

Findings
Two deficiencies were found related to improper storage of medications and toxic materials, posing risks to residents. Plans of correction were provided for both deficiencies.

F — The manager failed to ensure medications were stored in a separate locked area, as medications were found unlocked in a refrigerator in the garage with the door unlocked. This posed a risk to residents who could access the medication.
Poisonous or toxic materials — The manager failed to ensure toxic materials were stored in a locked area inaccessible to residents, as several toxic products were found unlocked in the garage and backyard with doors unlocked. This posed a risk to resident health and safety.
