On-site annual compliance inspection of ACE Castles Assisted Living, LLC conducted on July 7, 2023.
Findings
Three deficiencies were found related to staff training and personnel records, including failure to provide fall prevention training, lack of CPR training documentation for a volunteer, and missing orientation documentation for employees and volunteers.

36-420.01 — The facility failed to develop and administer a fall prevention and fall recovery training program for all staff, posing a risk to resident safety. Personnel records for two staff members did not include required training documentation.
The manager failed to establish policies and procedures covering cardiopulmonary resuscitation training for volunteers, resulting in a volunteer without CPR training documentation. The policy did not require CPR training for this volunteer, who was not affiliated with a hospice organization.
R9-10-101.155 — The manager failed to ensure personnel records included documentation of completed orientation for two personnel members. Both were observed working without available orientation documentation.
