Visit Reason
On-site complaint investigation of complaint AZ00191684 combined with an annual compliance inspection at an Assisted Living Home, conducted 25 April 2024.

Complaint Details
The following deficiencies were found during the on-site compliance inspection and investigation of complaint AZ00191684 conducted on April 25, 2024.
Findings
The inspection found ten deficiencies related to service plan updates, signatures, vaccination documentation, medication storage, and safety practices. Plans of correction were provided for all deficiencies.

The manager failed to ensure a written service plan was updated at least once every six months for a resident receiving personal care services, posing a health and safety risk.
The manager failed to ensure a written service plan was updated at least once every three months for a resident receiving directed care services, posing a health and safety risk.
The manager failed to ensure written service plans included the signature and date from the resident or representative for two residents, posing a health and safety risk.
