Visit Reason
The Office of the State Fire Marshal conducted a scheduled fire safety inspection at the facility to assess compliance with fire and life safety codes.
Findings
The inspection identified multiple fire safety violations including issues with appliance restraining devices, fire door operations, sprinkler and fire alarm system testing documentation, fire extinguisher maintenance, fire department connection clearance and testing, means of egress obstructions, emergency lighting failures, and delayed egress door malfunction. The facility was disapproved due to these unresolved deficiencies.
Deficiencies (11)
IFC 606.4 (2021) - The gas appliances on casters in the kitchen are not limited by a restraining device as required by the manufacturer's installation instructions.
IFC 701.6 (2021) - Facility is unable to provide documentation that the annual fire resistance rated construction material inspection has been completed.
IFC 705.2.4 (2021) - Resident room fire doors and the memory care dining room fire door would not close and latch from the fully open position; double fire doors lacked a door coordinator to ensure proper closing and latching.
IFC 903.5 (2021) - Facility is unable to provide documentation for annual sprinkler system inspection, 3-year dry system full flow trip test, annual dry system trip test, annual forward flow test, and protective covering on the water flow switch has been removed.
IFC 906.2 (2021) - The required annual maintenance for the fire extinguisher in the elevator equipment room has not been completed in accordance with NFPA 10.
IFC 907.8 (2021) - Facility is unable to provide documentation for the annual fire alarm system testing.
IFC 912.4.2 (2021) - Shrubbery is obstructing the required 36-inch working clearance around the fire department connection.
IFC 912.7 (2021) - Facility is unable to provide documentation that the fire department connection has been hydrostatically tested in accordance with NFPA 25.
IFC 1003.6 (2021) - Retractable 'do not enter' ropes were installed blocking access to emergency exits in the 2nd floor stairwell near room 202 and in front of the emergency exit door in the living room.
IFC 1008.3.1 (2021) - The emergency egress light near room 318 would not illuminate when the test button was pressed.
IFC 1010.2.13 (2021) - The delayed egress door at the main entry of the memory care unit did not open within 15 seconds as required.

Visit Reason
The Department completed a follow-up inspection of the Assisted Living Facility to verify correction of previously cited deficiencies.
Findings
The follow-up inspection found no deficiencies and confirmed that all previously cited deficiencies were corrected. The facility meets the Assisted Living Facility licensing requirements.

WAC 388-78A-2210 Medication services. The facility failed to administer oral and topical medications as prescribed for one resident, placing the resident at risk for compromised health.
WAC 388-78A-2130 Service agreement planning. The facility failed to update the service plan for two sampled residents, placing them at risk for unmet care needs and diminished quality of life.
WAC 388-78A-2120 Monitoring residents' well-being. The facility failed to monitor one sampled resident's blood sugar levels and take appropriate action, placing the resident at risk of medical complications.
WAC 388-78A-2703 Safety of the built environment. The facility failed to ensure that one door to the secured memory care outdoor courtyard was locked, allowing unrestricted access and placing residents at risk.
