Visit Reason
Unannounced routine inspection to review compliance with assisted living facility regulations.
Findings
The inspection identified 17 rule noncompliances across various regulatory requirements including personnel training, resident assessments, medication administration, emergency preparedness, and housekeeping. Several deficiencies were noted related to staff training, resident service plans, medication security, emergency response plans, and record keeping.

R432-270-9(7)(a)-(f) Two employees did not have core competency training in file and one employee did not have 16 hour one-on-one training.
R432-270-9(9)(a)-(l) One employee did not have department approved core competency in file.
R432-270-13(1) Two resident assessments were not completed.
R432-270-14(3)(b) Four resident service plans did not include who will provide the service, how services are provided, and frequency of services.
R432-270-19(16) Medication errors were not incorporated into the facility quality improvement process.
