Visit Reason
On-site complaint investigation and annual compliance inspection of complaints 00138507 and 00104282 at an Assisted Living Home, conducted 5 August 2025.

Complaint Details
The following deficiencies were found during the on-site compliance inspection and investigation of complaint 00138507 and 00104282 conducted on August 5, 2025.
Findings
This inspection found seven deficiencies related to facility modifications without approval, tuberculosis training documentation, incomplete service documentation, medication storage, toxic material storage, resident privacy, and employee in-service education.

Deficiencies (7)
R9-10-110 — The licensee implemented a modification of the facility by splitting the primary bedroom into two separate bedrooms without Department approval or submission of an updated floor plan.
R9-10-113 — The facility failed to document annual tuberculosis training and education for two employees, posing a potential illness risk to residents.
R9-10-808 — The caregiver failed to document services provided in residents' medical records for three of four residents, risking verification of services against service plans.
R9-10-817 — Medication was not stored in a separate locked area, with various medications observed in common areas accessible to residents.
R9-10-820 — Poisonous and toxic materials were stored in unlocked areas accessible to residents, including air fresheners, antifreeze, and cleaning chemicals.
R9-10-821 — A resident's sleeping area was used as a passageway to a common area and bathroom without a valid exception, violating privacy rights.
R9-10-803 — Policies and procedures covering in-service education were not implemented for two employees, risking lack of organized instruction related to physical health services.
