Findings

Findings
The inspection found five deficiencies related to personnel record compliance, verification of caregiver skills, and swimming pool safety requirements.

Deficiencies (5)
A governing authority failed to ensure compliance with A.R.S. § 36-411(C) for two caregivers, lacking documentation of good faith efforts to contact previous employers and verify fingerprint clearance cards. This is a repeat deficiency from a prior inspection.
A manager failed to verify and document the skills and knowledge of two caregivers before they provided physical health services, contrary to policies and procedures.
