On-site annual compliance inspection of an Assisted Living Home conducted on August 6, 2024.
Findings
Four deficiencies were found related to documentation of resident acceptance requirements, vaccination notification, evacuation orientation, and storage of poisonous materials. The facility provided plans of correction for all deficiencies.
Deficiencies (4)
The manager failed to ensure that documentation dated within 90 days before acceptance included whether a resident required continuous medical services, nursing services, or restraints for one of two residents reviewed. The missing information posed a risk if the facility was unable to meet the resident's needs.
A resident's medical record did not contain documentation of notification of the availability of influenza vaccination as required by A.R.S. § 36-406(1)(d). This posed a potential illness risk to residents.
The manager failed to ensure documentation of a resident's orientation to the facility's evacuation plan and exit routes was available for one of two residents reviewed. This posed a health and safety risk in an emergency.
Poisonous or toxic materials were stored in an unlocked kitchen drawer accessible to residents, posing a risk to their physical health and safety.
