Findings
The establishment was found to have multiple deficiencies including failure to ensure appropriate resident placement, incomplete fire drill procedures, lack of on-duty CPR-certified staff at all times, incomplete service plan updates for residents, and inadequate dementia training documentation for some staff.
Deficiencies (5)
A2000 (295.2000 Residency Requirements) - The establishment failed to ensure that two residents were appropriate for assisted living, resulting in inability to provide skilled services for one resident and inability to prevent elopement and wandering for another.
A2040 (295.2040 Disaster Preparedness) - The establishment failed to identify residents needing assistance for evacuation in fire drill evaluations and failed to involve actual evacuation of all or majority of residents to provide experience using various means of escape.
A3000 (295.3000 Personnel Requirements, Qualifications and Training) - The establishment failed to ensure at least one direct care staff person on duty at all times had current adult CPR certification including demonstration of ability to perform CPR, as four of eight employees had only online CPR training.
A4010 (295.4010 Service Plan) - The establishment failed to review and revise service plans with interventions to address a resident's history of elopement and wandering, a resident's fracture and pain management, and coordination of outside therapy services.
A4060 (295.4060 Alzheimer's and Dementia Programs) - The establishment failed to provide documentation of 16 hours of on-the-job dementia supervision and training for two employees out of eight reviewed.
