Surveyor conducted a complaint investigation and abbreviated survey at At Home Again Columbus LLC, a CBRF in Columbus. The complaint was unsubstantiated.

The complaint was unsubstantiated.
Findings
Five deficiencies were identified including failure to complete caregiver background checks every 4 years, improper disposition of medications with expired insulin being administered, use of semi-rigid dryer vents instead of rigid material, failure to conduct semi-annual emergency evacuation drills, and water temperatures exceeding the allowed maximum of 115°F in resident bathrooms.

Provider did not ensure a background check was completed every 4 years for 1 caregiver who had worked for more than 4 years.
Provider did not ensure medications that had been changed, discontinued, or expired were removed from the medication cart and disposed of within 30 days; Resident 1 was receiving expired insulin.
Provider did not ensure 3 of 5 dryer vents were made of rigid material; semi-rigid vents were used instead.
Provider did not ensure other evacuation drills such as tornado or flooding were conducted at least semi-annually; only 1 drill was completed in 2024 and none in the first half of 2025.
Provider did not ensure water fixtures used by residents did not exceed 115°F; water temperature reached 137.1°F in Resident 2's bathroom.
