The following deficiencies were found during the on-site compliance inspection and investigation of complaint AZ00188144, AZ00193810, AZ00197416, and AZ00201873, conducted on June 11, 2024:
Findings
The inspection found one deficiency related to failure to provide evidence of freedom from infectious tuberculosis for personnel. This posed a potential TB infection risk to residents.
Deficiencies (1)
R9-10-113 — The administrator failed to ensure personnel provided evidence of freedom from infectious tuberculosis, lacking documentation of a required second tuberculosis skin test for one employee. This deficiency affected five of five sampled personnel members.
