A2040 (295.2040 Disaster Preparedness) - The establishment failed to conduct six bimonthly fire drills and tornado drills on each shift during February, creating a substantial probability of harm to all residents.
A3000 (295.3000 Personnel Requirements, Qualifications and Training) - The establishment failed to ensure health care worker background checks, nurse aide registry checks, employee orientations, and fire extinguisher/disaster training were completed for five employees reviewed.
