Findings
The establishment was found to have multiple deficiencies including failure to meet residency requirements for a hospice resident, incomplete and outdated service plans for residents, and inadequate dementia-specific staff training.
Deficiencies (3)
A2000 (295.2000 Residency Requirements) - The establishment accepted for residency one resident who requires extensive assistance with activities of daily living including bathing, toileting, oral hygiene, and dressing while on hospice care without meeting residency requirements.
A4010 (295.4010 Service Plan) - The establishment failed to revise the service plan to address an unwitnessed fall, use of oxygen therapy, and responsibility for care and maintenance of medical devices for residents reviewed.
A4060 (295.4060 Alzheimer's and Dementia Programs) - The establishment failed to ensure that 3 of 8 newly hired employees received required dementia-specific orientation training, 3 newly hired direct care employees received required on-the-job supervision and training, and 2 employees employed over 12 months completed required annual dementia training.
