The follow-up inspection found no deficiencies and confirmed that the facility meets Assisted Living Facility licensing requirements. Previously cited deficiencies were corrected.

Complaint #176560 investigated the cause of a fire alarm activation. The cause was battery failure with no fire or injuries. No violations were found.

The investigation found that the fire alarm activation was caused by a battery failure in a nursing paging system that was no longer in use. No violations were noted during the inspection.

Complaint #103822 alleged a power outage and generator malfunction. The complaint was investigated and found unsubstantiated as no fire or sprinkler activation occurred and no evacuations or injuries were reported.

The investigation found no signs of fire, the sprinkler system was not activated, no evacuations or injuries occurred, and the fire department did not respond. A small leak on the generator's anti-freeze line was repaired and is now in working order.

Complaint #63659 involved a sprinkler system leak/burst. The allegation was investigated through document review and interview with the Administrator. No violations or fire damage were found, and the complaint was effectively addressed with repairs and fire watch.

The fire alarm was triggered by waterflow from a broken sprinkler pipe causing flooding confined to the front offices and lobby. There was no fire, no tripped sprinkler heads, no evacuation, and no injuries. The water was shut off and fire watch was maintained until repairs were completed. Restoration work was ongoing at the time of inspection.

Complaint #59414 involved a dry sprinkler system taken down for repairs. The technician diagnosed and repaired faulty components, completed repairs, and the fire watch ended without incident.

The fire alarm panel reported a supervisory alarm for low air on November 22, 2022. Repairs were made by a technician the following day, and the fire watch was completed without any fire or sprinkler heads tripped. There were no evacuations or injuries, and the local fire department did not respond.
